Overview of NC InCK's Structure: Care Management and the Alternative Payment Model (APM)
The NC InCK model includes enhanced care management and an alternative payment model to support the delivery of high-quality health care for children and the medical providers who deliver these services. Children in NC InCK are eligible for pediatric care management provided by specialized teams through their prepaid health plans (PHP) or health systems (e.g., Duke, UNC, or CCPN). The NC InCK APM includes six performance measures designed to enhance whole-child health. The NC InCK program is funded until December 31, 2026 with possible statewide expansion pending evaluation of outcomes.
NC InCK's Care Management
-
- Child-focused risk stratification to assign children to Service Integration Level (SIL) based on holistic cross-sector data indicators.
-
- Children in SIL 2 and 3 (~15%) are offered enhanced care management from the NC InCK care manager called a Family Navigator.
-
- Family Navigators are based within each health system's or PHP's care management team. They work with a family to facilitate integrated care for a minimum of one year.
-
- Family Navigators work together with families to completed a Shared Action Plan (SAP), a collaborative care plan led by families to communicate goals across a child's care team.
NC InCK's Alternative Payment Model
-
- Six child-centered performance measures prioritize connecting children to quality services
-
- Health systems are eligible to receive additional financial resources with achievement of APM measures
-
- Health systems will receive data reports that summarize progress on APM measures. They will also receive reports on additional awareness measures, which are not linked to incentive payments but are shared for awareness purposes only."
-
- There are no risks associated with participation and no penalties if targets are no achieved.
Health Care Providers and the NC InCK Care Model
-
- Family Navigators can help health care providers by connecting and coordinating available community resources for patients based on their identified needs.
-
- Families may identify health care providers as part of their child's integrated care team, which may also include representatives from their child's school, juvenile justice, child welfare, and/or other family members or friends who support the child.
-
- Health care providers will have access to their patients' Shared Action Plans if a family gives consent, and they may be invited to participate in integrated care team meetings.
Health Care Providers and the NC InCK Alternative Payment Model
-
- Health care providers and practices participating in the APM will have implementation support from both their health care system and NC InCK. This support may include:
-
-
- Assistance with identifying local community resources
- Guidance for new coding and billing processes
- Invitations to participate in collaborative learning meetings
-
-
- Success with the APM will lead to financial incentives that may help sustain the resources needed for high quality care and address social drivers of health.
-
- In addition to incentivized measures, health care providers will receive actionable data on indicators such as the rates of kindergarten readiness and chronic absenteeism for children in their practice.
